Vendor Approval Process

If you wish to become a TEACH-NW vendor, please complete the following process:

  1. Complete a Vendor Approval Form.

  2. Using the ‘Add File’ link, please attach a completed W-9 and Proof of Qualification (i.e., Business License, Diploma, Certification, Resume).

  3. Tutors (including teachers lacking a current certification) must complete a background check. Once the form is received, we will provide you with the necessary background check documentation.
